GetOut! Report Back Form (revised for Phase Two projects)

This form should be filled out by all GetOut! grant recipients & new recreation programmers at the completion of their projects. The Project Report Back is one way to share what you’ve learned with us & with others who are organizing similar projects around Vancouver: your successes, challenges, stories & remaining questions. Please download this form & fill it out electronically—as honestly as you can. Tell us about all the great things that did work, but also be sure to include what might not have worked so well…so we can all benefit from the lessons learned & continue to learn together about how to reach out to Vancouver’s diverse young people & help them get out & get active. GetOut! Appearance Release Form

If you are taking photographs, video or audio recordings to document your project, it is important to have those appearing in the photos or recordings to sign an “Appearance Release”. This release gives you permission to use their photo, video footage or audio recording in promotional materials, news articles, report-backs to funders, and other project documentation. It is important that you have a signed appearance release form back from everyone who is recognizable in a photo, video or audio clip, before you use it. If the participant is under the age of 18, you also need to get their legal guardian to sign the form. When using this form, remember to change the text in blue to suit your project before printing it out. GetOut! Participant Survey Template (revised for Phase Two projects)

This is the template to develop a survey for the participants in your GetOut! projects. The answers to these questions will help us find out if GetOut! is achieving its goals and aspirations in reaching a diversity of Vancouver youth and helping them GetOut! and GetActive! When working with the template, please keep the original questions on the survey as they are, but feel free to add your project-specific questions. If you have any questions about the survey or survey development, or if you would like to modify one of the original questions on the template, please contact Steven at steven@heygetout.ca. After your participants have filled out the survey, contact us at the same address and we can arrange to have them picked up.
